By signing this form, I acknowledge and fully understand the following:
I am voluntarily participating in the Halloween Pole Show 2025, an independently organized live pole dance and performance art show, taking place at Bain Mathieu, a unique and historic venue (formerly a swimming pool) with special structural features including deep stage areas, stairs, and backstage space.
I understand that this year’s show includes:
A 14-foot pole mounted to a truss rigging system
Use of professional-grade lighting and sound effects
A 360° audience layout with no traditional backstage
Variable surfaces and an immersive environmental design
I acknowledge that pole dance and performance arts involve inherent risks, which may include (but are not limited to):
Falls from height
Muscle strain, sprains, fractures, joint dislocations, concussions
Cuts, bruises, rope/fabric burns, or other surface injuries
Equipment failure or malfunction
Exposure to fog, flashing lights, or loud music, which may affect visibility or coordination
Distractions from the audience, slippery shoes or costumes, and unexpected contact injuries
I freely assume all such risks, including those arising from negligence, and understand that in rare but possible scenarios, these risks could result in serious injury, permanent disability, or death.
I confirm that I am physically and mentally fit to participate and have consulted a medical professional if necessary. I accept full responsibility for my actions and safety throughout my participation in the event.
I hereby waive, release, and hold harmless the organizers (Margaux Brissiaud, Cecilia Lico), Calmness Hotline, Mtlpoleshoots, their collaborators, crew, volunteers, venue staff, and sponsors from any and all liability, claims, demands, losses, or damages — including those related to personal injury, property damage, theft, or death — whether caused by negligence or otherwise.
I understand that this waiver is binding and applies to all participation in rehearsals, test runs, and the live performance itself.
